Completed C1 Form (Adult)
or Completed C2 Form (Child) and LS01 A Garda needs to stamp the Lost or Stolen Application Form. You will need Adobe Acrobat software to download these forms & notes. If you do not have Adobe Acrobat installed on your PC, please click here to download. NB: Applications to replace a lost or stolen Passport may take longer to process as record checks will be carried out. |

SIGNATURE:

Ensure you sign within the signature boxes and initial where requested on the C1 or C2 forms
Please be advised, failure to comply with this instruction will invalidate your application. |

PHOTOGRAPHS:

2 current passport size (one must be signed by a professional person resident in the Republic of Ireland. The same person needs to sign Section 10 of the C1 or Section 8 of the C2 form)
NB: Stringent guidelines for passport photographs are now in place and unacceptable photographs will delay the processing of applications. click here |

Please note: If you have had two previous lost/stolen passports your new passport will be restricted to one year and a full passport fee is required. When renewing this one year passport, a fee for a ten year passport will again be required.
